So I managed to get a new moniter, went from a 1368x768x60 to a 1920x1080x144, and it worked perfectly for the first 2 days, then this started to happen.

I now have to restart my computer a couple of times for it to work, I'm using a AMD R9 260 I think, unfortunately I'm at work until this afternoon, will update with full specs then, hoping someone know something of the top of their head by then.

Thanks in advance.

Hmm, you might have a faulty GPU, but try above first.

What's your system spec including PSU make and model.

I had something similar which looked like this https://i.sli.mg/xSQBab.jpg

but a total freeze with audio looping and only when in a game for a few mins, it turned out that it my GPu was overheating , needs new thermal paste.

check temps just to be sure, but it's likely a heat thing upon booting windows.

check your cable too, and swap the card into another PCIE slot, even if it's 4x 8x.

If you have integrated graphics try that.
