Elite dangerous multiplayer options

Would I be right in thinking that the only multiplayer option is to join a server which has all players on it? There's no way to have a set-up with just friends present?

No, you can also join existing groups, or play exclusively with your friends.

EDIT: Having done some deeper googling based on your answer, I see that the servers run by the game makers allow you to use their own and make one private. Similar to GTA5 I guess. That's perfect. Thanks!

Nah, you create a private group and then it will only instance you with your friends

In many ways you are still part of the bigger game world but you can limit who you can and can't encounter if you wish.
