I have a MSI GTX660 and only recently cleaned out my PC to find the GPU had accumulated a lot of dust underneath the fan.
I looked in Afterburner and messed around with the fan curve to find it wouldn't run anything below 84% RPM.
For example, if you check out the image, the fan won't actually turn on until around the 55C temperature. Anything below 55C and the fan won't actually spin!

I then tried different temperatures to find it roughly doesn't spin on any RPM below 84%.
Any ideas?
I already changed the BIOS back to stock and cleaned out all the dust.
The fan runs at 4650RPM when on full load and throttles down to 0RPM once it goes below 55C.
