What could be causing this please, any idea on how to troubleshoot?

It says your average gpu frequency was 536 mhz, looks like the gpu is the problem.

Maybe a power issue and it's not getting enough power?

Just so you know I think I was about 12k with a 3900xt and a 6700xt

Download GPU-z and check you are running in PCIE 4.0 X16 mode. Your GPU clock speed is throttling massively for some reason, what is the peak GPU temperature and the peak CPU temperature? Download HWInfo to check the voltages for your GPU and check that power supply is providing 12V to the 12V rail. If the graphics card needs multiple power connectors make sure they are all plugged in correctly (I would unplug and plug them back in) and make sure you are using separate PCIE power cables, not one cable with two heads. Check the graphics card is fully seated in the slot.
