I'd agree with the one lane in/one lane out so its a single lane roundabout and you'd be at fault although with insurance nothing is ever cut and dry it seems (never had to make a claim myself). Maybe its wider than a single car but not marked in 2 lanes as this would allow wider vehicles eg lorries, buses more turning space around the roundabout.
I dont know how long you have been driving but the most basic rule is to treat every other driver as a clueless idiot and assume nothing. Seems from the post in the thread that you assumed she was going straight on when she wasnt.
