Spread it and then put a blob in the middle, best of both worlds.
Not really, ideally you want to use as little TIM as possible whilst maintaining the full coverage of the heatsink.

I think some people tend to over analyse things, all that nice spreading around will get shot to hell the moment you cram that massive heatsink on it.
All you need to do is put a nice splodge in the middle, slap on the heatsink and get on with the more exciting parts of the build.
None of my CPU's have exploded, combusted or china-syndromed doing it this way.
