Build muscle and lose fat at the same time ?

Just 2 questions which I have tried to Google but failed so just want a lay-mans explanation -

What IS lean muscle or lean muscle mass?
How is it diff from "normal"? muscle or muscle mass

It's not, muscle is muscle. They simply mean to create a tendency for weight gained to be muscle weight not fat.

Imagine a man working out at the gym eating a huge cal surplus on big macs and fries or whatever. You come back to him after a set period of time and he's put on 20lbs. 10lbs muscle 10lbs fat.

Different guy has focused on a slower but cleaner bulk, he's only gained 11lbs but because he's taken care of what he eats and only allowed a smaller surplus each day in calories 9lbs of that is muscle and only 2lbs is fat.

Muscle is lean so saying I gained 3lbs of muscle or I gained 3lbs of lean mass is the same thing (or is implied to be the same thing)

What people mean when they gain "lean mass" is that they've gained weight without gaining bodyfat or water %. Which 99% of the time is a lie :D you'll always have water and bodyfat fluctuations.
 
This is possible, it just depends on your diet, work out and the condition of your body. I believe I'm doing this but on a small scale but then I'm not trying to put on as much muscle as possible and don't require huge calorie intakes.

I've always questioned people when they've said you can't build muscle and loose weight. I think DM has summed it up well, as long as you're maintaining a high enough calorie intake including calories from body fat you can achieve this.

Lean bulking is such a tricky thing, i was gaining fine to begin with, but as the mass went on i found myself having to gradually eat more and more.

I always assumed that you always put on fat when bulking, it's keeping it to a minimum whilst still providing what the body requires.
