You shouldn't be using sleep or hibernate with an overclock. Happens with S.B chips as well. remove your overclock for a bit and I recon the problems will disappear.

I've not heard of this, why would you not let a PC sleep with an OC ?

It can't provide enough power back to it on startup -PLL I believe is the cause.

Ok I have a further update.
The issue developed again.
I have left my case open to see exactly what's going on when it happens.
This time I noticed, when i powered up the machine, the psu, hard drives and gfx card were going. However none of the case fans were going.
The next time, same devices again, as well as some, but not all the case fans.
My thinking now is that it's definitely down to the power supply, it feels like it's not delivering enough power on the 12v rail and I think the issue may relate to the 24pin/2x4pin mobo connectors and the power they deliver.
What do you guys think?

try with dram 1.55v cpu vtt 1.10v and disable pll overvoltage in bios,just test and see if it resumes properly,

strip the system down to the bar minimum detailed below and see how it runs.
unplug power to anything thats not needed drives, fans etc
unplug the sata cables from the mobo and any case fans
remove the graphics card after setting the system up to run with onboard graphics
leave 1 stick of memory in
if that runs ok then start to rebuild the system
add 1 stick of memory at a time, see how it runs until there all in.
once all the memory is in reconnect case fans 1 at a time til there all done
repeat with any drives
finally put the gfx card in.

It'll be the PSU. If you're 100% sure it's not cpu/mobo/ram it will be the PSU half the things you've said would never happen if the GPU was giving you problems. The PSU is old it needs replacing anyways get a 80+ bronze one or above for peace of mind about it's quality.
Just ordered on OCuk, the coolermaster silent 600w modular.. Hopefully this'll fix the issue!
I've got the 700W variant. You need to press the sata power cables in VERY hard with first use. Apart from that it's spot on.
