How to Generate Online Surveys

Right, I have my own online shop, but I would like feedback from people after they buy. Is there anywhere I can generate a survey to be hosted, I send them the link, they fill in some questions (With a coupon code for money off next purchase if its filled in) and then I get emailed / website generates results?
Anyone recommend any? I have minimal programming experience, only what I have picked up while upgrading and minor hacking of my website/forums/shop. (By hacking I mean modding of course!!).

Sounds like Limesurvey is just what you are after. I've been using it for a long time now and it's never let me down yet and I generally have to make some really awkward surveys for my customers.

The communities great and and very helpful if you use it. I can't say I've had any issues so far with it, as it's very easy to use. Sounds like I work for them I know (I don't) but I really do think it's great for an open source package. You can tailor it to match your current site by using the template editor as well.

Not sure how the coupon code would work, but you can set the exit page of the survey to forward them to a static page with a code on if that's any use?
