Metro 2033 is a close second to the Stalker trio IMO, weapons & ammo are very scarce & the atmosphere is very oppressive. A bit short but well worth a run through if you haven't played it & it'll give your system a run for its money!
 
Maybe I'll give it a go during the summer, I've seen plenty of videos of it and it does indeed look good :)

@Swi1ch: Have you found a game which met your requirements ?

I got the latest alpha release of Project Zomboid which is about as close as any game today gets to proper survival. It's genuinely nerve-wracking, and the longer you play the more tense it gets. You have to be constantly aware of what you are doing and plan your moves. Really exciting stuff, especially for an alpha.

I would definitely argue that Minecraft is not survival in any way at all, as you can entirely negate any danger in the first twenty minutes by building a wall around you, putting up a torch and planting some seeds. It's a sandbox with mild RPG elements.

I think my idea of survival seems to differ from other people's. For example I think Fallout NV with hardcore mode is an RPG with survival elements, as it's still fairly easy to become borderline superhero unless you intentionally gimp your character. You're not having to struggle to survive constantly, as you can hang around pretty safe places if you want. In Project Zomboid, it's always dangerous.

I also don't think a game that has horror elements can be described as survival just because there's monsters like Metro 2033. There's sort of ammo management in it but for the most part it's a linear shooter. I guess you're 'surviving' by not getting killed but I don't feel like it's the same thing.

STALKER COP Misery Mod looks excellent. STALKER Complete was very close to pure survival before, but this looks like it's been taken up a notch. A couple of the features I really like include:
-Seek a more natural day/night rhythm without drugs if you want to avoid a blurred vision caused by headache or the need for recreation and rest.
-Low weapon and ammo resources. No more "killing zombies to easily find loads of weapons and ammo". - This is a huge deal for me. What I dislike about Fallout is you see a cave and you have to consider the consequences of going in it, which is basically resource consumption and potential damage. Thing is, you know damn well that there are shiny things in that cave, or big bads who will give you lots of XP, or a quest etc, which means that the hardcore mode is simply a difficulty increase. Once there might not be loot down there it becomes much more of a difficult decision.

Overall the mod looks amazing. The only thing i'd like to see in it (It's not listed in features but might be ingame given the tone of the project) is reloading by magazine rather than a stash of bullets that magically fill your weapon when you reload. The current system means that you choose to carry as much ammo as you can whilst giving yourself enough carry space left for loot, so you might take 300 rounds of ammo, with each round weighing x. I'd like to have to pick how many magazines to take as well as actual rounds, adding further depth to your reloading habits.

Yes, I know I ask a lot of games :D
