I *think* the picture is of the loco after the incident.
This has been discussed elsewhere, and from what I remember the piston/ cylinder would basically have come out the top at a slight diagonal, and these are only bonnet type flaps with a catch. Would either have popped the catch or made a hole in the top where you can't see it :)
The cylinders are a separate casting bolted on, hence why the whole thing hasn't destroyed itself, just one cylinder.

Nice projectile.

Can't help but think it would have done a lot less damage to a proper house instead of that typical north american cardboard one ;)
 
Not quite as dramatic but if you search for Class 60 Isambard Kingdom Brunel, you'll probably find pics of an "uncontained failure". Quite an impressive force.


I used to repair pressure washers and whilst working on the most powerful pump we'd ever received (powered from a monster 3 phase motor) we didn't use an unloader valve and just taped the gun. The tape got wet and came off just as a workmate had his head over the pump. One of the valves just brushed his hair, went through the roof and landed in the car park. It would have killed him easy.

Big bits of flying metal are dangerous folks :)
