New user account vista

Should be as simple as just creating the new account.

Although you'll have to set everything up again, like your sidebar and what not.
 
Games and programs installed on the system are fine, as long as they are not installed into the old user accounts folders it will be fine as when you remove the account you will delete the game.

as long as the game is installed into say the C:\Program Files not C:\Documents and Settings\[OLD ACCOUNT USER]\

Depends on the app and how you installed it if it asked you the question 'install for this user only or install for all users', or what ever it is it says.

All depends which hive of the registry the keys get put in.

Set up the new account, Jim, and while logged in as the old Jimmy account copy over the contents of .\users\jimmy\ folder to .\users\jim\ folder. May need to log in as third account if you run into locked files. Just make sure what ever account you do it with has full rights to both.

Also do not delete the old account, or data, straight away. Give it a few days, week or more, until you are 100% happy that you aren't going to lose anything.

As for the registry issues I mention above you could get into playing with the registry to move all that over but you'll most likely bring over any issues you are trying to fix and probably create more. So either live with reinstalling a few apps here and there or do a total rebuild.
