id engines are great, since they allow you to use absolutely any resolution you like (providing your gfx/monitor driver supports it).
 
That's what you put in the console.
You could also edit the cfg (without the vid_restart) but I can't remember the directory structure for Quake4. It'll probably be in a "base" subdir or something.

And no, you don't touch the res in game.
 
Is it normal for me to be getting any sort of lag or mouse lag when at 1680x1050 with 16x Adaptive AA/16xAF enabled?
