Squeaky brakes and grinding sound

This is sort of prompted by Fox's thread, my brakes squeak almost constantly under braking and at about 2.5k rpm there is a faint grinding sound which appears to be coming from the wheelarch (although difficult to pinpoint when driving). Sounds like the pads/discs need replaced?

more than likely, look and see if the pad material has all gone.
