Turned on PC from sleep and was greeted with this

That's the gpu driver crashing and recovering.
First port of call is software, specifically the driver software. Update it.
The image in the screenshot, as well as the driver crashes, if not caused by software, are hardware. Possibly power, but most likely a faulty gpu.
