Probably the fact that name, location and email are required fields that's putting people off.

Why not make them optional? They're not relevant at all to the information you need to collect. If people want to apply for the prize give an option just for email. I'm sure people would be far more willing to fill a questionnaire if they didn't have to provide unnecessary personal information.
 
What do you need the name, address and email for then?

I could maybe understand the address if all you wanted was a rough idea of where each person is located, but that's not what you're asking for.

Thanks Maccy. Appreciate that. Promise I will stay around. Most people have been really supportive on here.

The address is optional and you don't need to give it. A location is fine. I could take the name off but I don't feel it is an infringement on peoples security. Most people have just put their first name. And thirdly I need the email to inform the winner of the prize.

Maybe you have a point about the address fields.....even though they optional I will see if I can remove then.
