I once caused a car crash (so say the police, anyway) and was terrified I'd killed the occupants of the other car. Fortunately they were OK.

Car accident, driving down a hill but at the bottom of the hill the entire road had giant stone chippings as it was being redone. Promptly the wheels locked and i slid sideways into a ditch before being knocked out. Woke up in my car perfectly aligned on my side of the road but upside down.. No idea how i slid in and out of the ditch.
Anyway the police automatically assumed I was driving dangerously :S Phoned up to give them a piece of my mind for not sign posting it, he basically said it was as he had checked early that day, I asked him if he had a camera in his patrol car he said yes but completely blanked me about evidence. Couldnt be arsed with it in the end and just left it. 4 weeks of chilling at home playing MGS on release though
When I was younger I had that swing ball thing at my aunts house. My brother was messing about in the backyard and we were playing like kids do. For some reason the swing ball looked like a javelin to me (spike on the end) and I shouted to my bro that I was throwing it... It didnt go quite as far as i thought it would and hit my bro on the head. I thought Id killed him, I was in complete shock. Luckily it was just a couple of stitches at the hospital.
Ive also been in the situation of being in a fairground ride and hanging onto the safety bars for my life. Not very pleasent.
Absailing down the side of a cliff near the sea when I was young. I didnt have a problem with it untill the older guy (looked like a hardcase too) infront of me broke down crying saying he wouldnt do it... Then the nerves hit me, but I managed it in the end (im also scared of heights)
Probably some others but I think I embaressed myself enough for now

The moment I was riding my motorbike and a car decided to pull out infront of me...... it hurt!
Second most scary was the few seconds after as the stupid woman driving didn't register what she did until 1/2 her car was over me and the bike - didn't register how bad my injuries were.
One stuffed knee and that was it....... VERY lucky.
Oh, and not being able to contact my father who was in London on 7/7 - he was at a conference and the silly woman at the venue refused to give out the information as to whether he had signed in......... even with all that had happened that morning!

sadly, it did. i thought he was going to ask me for a lighter or something, so as i was walking away from the bar he grabbed my arm with my wallet in hand, said "give it me or i drop you", **** myself and gave it to him... pottered off to find my friends, who all said tell the bouncers, but this guy was looking at me from across the room so thought it was best just to stay put for a bit. he buggered off, so i went and told a bouncer and got the **** out of there.

Dad collapsing with a Cerebral Aneurysm caused by a mixture of twisted blood vessel and high blood pressure from stress.
By the time he was in hospital his vitals were 250/180.
The resulting haemorrage affected his speech, cognitition and personality.
That was a fun Christmas.
He's relatively fine these days which I'm very thankful for.

Nothing terribly bad (illness-wise) has ever really happened to me. But the worst fear I've ever had was rolling through a forest at about 3AM with some buddies after smoking... (can I say that? I don't know) and I was just walking across a gangwalk talking, and one of my friends leaped out of the bushes into my path and it scared me to the point of collapsing.

My wife and I used to commute from Brighton, me to Horsham and her to East Grinstead. Both share the same route up the A23. One day she left a few minutes before me. When I got half way up the A23 I got stuck in a traffic jam, so pulled off and took the back roads to my office. Knowing that she didn't have a mobile and would need to stick to the main road, I called her work to let them know that she would be late. The person who answered said, "Oh, we've heard that she's been involved in a road accident. I think she's OK - let me ask the person who spoke to the police..." then disappeared for the longest 60 seconds of my life. She had relatively minor injuries, but the car was a write off.
Also when my 18 month old daughter was rushed to hospital with a suspected ruptured intestine. Thankfully it was "only" acute salmonella poisoning so no operation needed.

When my uncle who lived in the Caribbean decided to let go of the steering wheel of his car and let it roll at 20mph or so towards a cliff drop of about 500ft, all the while saying "this is the end my friend". I'm still here, so of course it was a joke and he stopped about 10ft from the edge, but even at 12 i did not think it was funny in the least.
No,ended up splitting up for 10 months.
