Witness Expense Claims

I am set to appear as a witness and need to take some time off work - I have been told that I can claim compensation for this.

Does anyone know what I have to fill out on the form? Do I need to get my employer to sign anything? Do I get a standard compensation amount or is it calculated from my normal wage??
