I'll go for Willem Dafoe's Elias being shot by Barnes in Platoon. In the context you could probably see it coming, but the manner in which he is despatched was still pretty shocking. (Of course he wasn't fully dead there and then but that would be splitting hairs). You could also say Barnes getting whacked at the end is fairly unexpected, given his was in mid-goad at the time and Taylor didn't bother with any pre-whack diatribe, he just did it.
True, but in the book he doesn't die. Either way he doesn't die in the film
