People on steam forums had it appear in their game list after

Shut down Steam
delete or rename clientregistry.blob in your Steam folder
Restart Steam

or email it as a gift to yourself then add it to you steam account

Regards the Bioshock 1 issue - why dont the purchasers of the 4packs talk to each other and gift each other (i.e. Tom gifts MidnightLamp and vice versa?)

According to their forums you do get bioshock 1 if you didnt already have it after a restart
