Medieval II Total War is the game I have played most, over 300 hours now at a guesstimate. However, a lot of that time has been spent playing mods, which probably doesn't count - that is, each mod should be counted as a seperate game...

Other than that, I can still play through Half Life 2 a couple times a year, although that's probably because we're still waiting for Ep3/HL3

Dungeon Keeper 1 (To a lesser extent 2) and Theme Hospital are the games that I will always come back to after a while, hell I am playing through Theme Hospital now

I've just started Roller Tycoon 2 which is awesome. Transport Tycoon is another great game and I'll add Championship Manager to the list as well
