I've got to tell my partner that its over and I'm moving out and I've been procrastinating all day as I don't want to hurt them.

It's got me thinking - how do you deliver bad news?

Get it printed on a t-shirt, then give said t-shirt to them as a present.

Or:

Bake them a cake, write what you need to say on the icing, but keep it short unless you make a really big cake.

Sit her down, talk to her face to face and straight up. I'm sure she'll have her questions. However, be firm with your decision and don't look back.

Wait until she's out and then write a note on a postit and stick it to the fridge before leaving. (If t's your place then change the locks and place said postit on the outside of the door)

I had to tell my Mum I had found the Dad she had been searching for for the last 18 years, but that he had died back in 1991.

I said I had something to tell her, asked her to sit down, pre warned her it wasn't good news, and then just told her in simple terms.

Your other half deserves the decency of you being honest with him/her, even if he/she doesnt want the same thing you do, don't be a wimp and just run off, or leave him/her a note, they deserves better than that. They are bound to have questions (especially if they haven't seen this coming) and you need to answer them.
