7950 GX2 box question

My step-up is going through and have to send in the card now
Would it be possible for someone to just list out all the things that came along with the box? I'm not quite sure and don't wanna forget something I have and end up being charged for that since i'm already paying for the loss of the plasti casing and the paper cover.

CD, two adapters, warranty/manual docs, vivo lead, s-video extension and a molex/PCIe power adapter.

unfortunately, looks like i'm missing one s-video extension and one of the driver cds
need to look harder for them. have no idea where they could be
good news is, one card is all ready to go
