Adding a simple upload field to a form?

Hi all I currently have a form where people can fill in details about their events/club nights.

This gets sent to my email.

I'd like to add a field so they can upload their flyers too. I want the flyers to be uploaded to a folder on my host and then the link emailed to me. Is that easy to do?
