Admin Password Issues - El Capitan

My Macbook Pro performed a routine software update last night but since resetting I've been having strange password issues.

When I first start up the laptop and login it accepts my admin password.

But once I've logged in, it won't accept it for anything else (e.g. accessing keychain). If I log out and try to log back in it won't accept my password either.

Anyone have any idea how I can fix this? I haven't changed my admin password since I bought the thing 18 months ago so I'm quite confused!

And If I try and change my password from within OSX it says the existing one is incorrect.

I rarely lose my sheet with this computer but this is ridiculous. How have Apple implemented such a **** up :D
 
This is ridiculous.

I've reset my password using:

Recovery Key from login screen
Terminal via the Recovery Mode.

Yet the issue persists. What on earth is going on?
 
Just in case anyone digs this up in a search in the future.

I ran the reset password via terminal in recovery mode. I set a NEW password (instead of the old one) for both the administrator and myself - seems to have done the trick.

FYI if you have File Vault enabled you need to decrypt the hard drive within recovery mode (it's fairly straightforward).
