Getting confused with all the different chipsets

I need an ITX board with Optical and a PCI-E Slot to take an Athlon x4 or Phenom II

help me please :D

An Asus M4A88T-I DELUXE would do the job, though your DDR 2 would have to be disposed of. :( Oh and it's So-Dimm board too, just for good measure. :D

What little else is out there is lacking features and looking a bit dated. I'm waiting for a mobo maker to produce an AMD iTX board that supports Sata 3/USB 3. It may be a long wait.

Look up J&W Minix785G-SP128M - it fits your requirements from what I can tell. A fairly uknown manufacturer, though, and the technology in it is getting on a bit. It should be fine with your case/cooler, though. Doh! Just double checked, and it only takes sodimms - laptop variants of ddr2 modules - compact, but probably not what you want. Apologies.
