It doesn't look like it was made in a 3d package as the edges are too imperfect on the tweezery things (unless someone has actually gone though the trouble of modelling it to look imperfect).
I would say it's a mash up of scanned drawing with some photoshop, hard to say though.
yes. However it could be made in something like pro desktop as you can see all the different components... Um guessing there was a jig used aka template and the other details added in.
