Heard Madvr can offer picture quality for Blu-ray's etc. What exactly does it do?

Basically A filter/display codec that runs in conjunction with your gpu and the video players app. As with anything you can tweak the actual display itself and then the video app (I use media player classic).
End of the day you'll get it looking good with one source and then it may not look as good on another. Very easy to get obsessed with picture quality so I recommend you give it a try and don't delve to deeply into how it looks. Get it good enough with one source and leave it.
