Back/shoulder blade issues

Well here is a bit of history on my situation. I was run over in a hit and run on my bicycle around 2011, landed on my back after scoring a perfect 10/10 on a forward double front flip over the handle bars and over a car bonnet, ended up with a minor dislocation on my left shoulder and a fractured wrist on my right.
I was referred literally straight away to a physiotherapist not 3 days after the accident, I told them I could barely move/get out of bed in the morning, let alone dress myself. The one time I did manage to get dressed properly I urinated all over myself because I was unable to pull my trousers down it was that bad.
When I asked for another referral they put me down for another one in 7 months time!

I will stop here, otherwise I'll end up getting seriously ****.

But cutting a long story short I never got the proper after care treatment for an accident which seriously messed me up, my shoulder joint on the left hand side protrudes out 3cm when compared to the right and always gives me a dull pain every now and again that can go all the way to the tips of my fingers.

I am however now getting the exact same kinda pain, if not worse, but in my right shoulder blade. Just breathing which puts a slight pressure on my rear rib cage (if you can call it that) leaves me in serious pain across the back of my neck, and a numb/dull sensation in my elbow and finger tips, not to mention the pins and needles... I hate pins and needles.
I am finding it increasingly hard to work (typing and lifting) and sleeping due to putting pressure on my back when breathing.

tl:dr I asked my doctor to refer me to a chiropractor and he said no in no uncertain terms, what can I do? and can I go straight to private and by pass him? I've been told I need his referral and I just can't ***** deal with this anymore :mad::mad::(:(
 
Get a second opinion? Worst case, go to hospital when it gets really bad. When they ask why you didn't see your GP, mention you did.

Also, pay for 1 private session for an evaluation from a professional. It will cost, but then get the referral and opinions from the professional and take it to your doctor.

Simples.
