'Bad Pool Header' BSOD

Hey guys, unfortunately I'm looking for some advice on the origin of a blue screen I just had.

I've been playing a game (Dark Souls 2, if it means anything) for a few hours today, no problems whatsoever as usual, solid 60fps framerate. And I decide to play a YouTube video on my second monitor while I play. I reboot DS2 with the video playing (I do this a lot) and for some reason the framerate won't go above 30fps, and frequently drops below, which is strange as my monitor actually runs on my CPU, and has never affected performance in a game before. Anyway I remember that I'd changed my hz frequency on my main monitor down to 120hz that day so I thought that might have something to do with it, but once I select 144hz in AMD CCC the screen goes black and a load of strange coloured letters lined vertically on the screen appear with a strange coloured background, the screen blinks on and off and the letters become very blurred. So I force shut down my PC and now I can't turn it on without getting a BSOD when I boot into windows. The only way I've managed to get it to work is by booting it in low resolution mode, then manually changing the resolution and hz afterwards. (This disables my second screen).

Sorry for the long and badly worded paragraph, I'm a little worried as my graphics card is just over half a year old and I'm scared it may be an issue with that. :(
