That box thing at the top covers the house roof straps I believe... or maybe a wooden beam.

Hole is too high to fit a usual extractor fan because they all come with bulky square surrounds.

Any ideas on what to do?

What's behind that hole? Is it possible to take a vent house up into the loft and out through the soffits or roof?

If so, rather than a wall-mounted fan, you could fit an inline fan (which are better anyway) and a small plastic shroud for that hole.

Either cut a bit out of it to make your extractor fit.

Or get something the same thickness. Cut it to fit your extractor fan. Stick it to the wall, and fix your fan to it.
