Best account and debit/credit/prepaid card for foreign travel?

I am looking for recommendations for a bank account with a debit or prepaid card which can be used in multiple countries with low fees.

Likely destinations, if that's relevant, are US, EU and (less likely) South Africa.

Thanks all. I've decided to open two accounts and seeing how they both fare; Wise and Revolut. I will take my Lloyds debit card with me as a backup in case of problems with those cards. Lloyds is a Visa Debit and Wise/Revolut appear to be Mastercard Debit. So I think I will have all bases covered with them.
