Best way to take meeting notes on an iPad?

Morning all,

This is a problem that should require only a tiny percentage of OCUK's combined brainpower to solve but where Google and a number of other sources have failed....

I want to create a standard meeting note template on an iPad app for our team at work with fields such as company met with, date, attendees, key items for discussion. Now this sounds simple but I've yet to find an app that can do this elegantly. The notes would ideally be in typed form as I find handwriting on an iPad frustrating.
There is a work around where you save a pdf in GoodReader and then "open in" a note taking app, thus preserving the oringinal pdf in GoodReader but this is clunky at best.

Any help much appreciated!

Create a PDF template with interactive fields.

Personally I'd just roll with a pen and paper, or better yet a Livescribe, but each to their own.
 
Thanks for the replies - we are now having to digitise all of our notes so either I write them with a P&P (which I find easier and quicker) and then have to type them up afterwards or just try to do it in one go.
 
I would still prefer to use pen and paper and then type up afterwards. Or have a wireless keyboard for the iPad.
 
Wireless keyboard is something I already have - doesn't solve the template issue though. Is an amazingly simple problem with no solution apparent...!
