BF3 hangs - graphics card related.

Hi!

Sometimes when I play BF3 it hangs and quits with the following error mesage:
DirectX function "GetDeviceRemovedReason" failed with DXGI_ERROR_DEVICE HUNG: Device hung due to badly formed commands. GPU "NVIDIA GeForce GTX680 Driver 33489"

Temps look ok certainly not too hot. BF3 is on Ultra. Has done this on just about every update so trying other drivers won't help
Sometimes I can play for ages, other times it'll crash on every map :mad:

Any thoughts?? Actual card is a KFA 680 GTX OC (white PCB)

I had a GTX570 that used to throw that exact error because it wasn't stable even at stock clocks on default voltage. It was cured by upping the voltage a tad but the card went back. As previous is the card overclocked?
 
It's a factory OC, not touched it personally. Am also running 2 monitors off this card I should say!
Just played it again and played 3 maps (crashed on 3rd map) with fans @ 50%, temp at 74 degrees....
Interestingly how does one use a monitor with a card and the 2nd off the onboard graphics?
 
SO how does one proceed? Try boosting the voltage as mentioned but can I use Afterburner or GPU Tweak even though its NOT an Asus nor MSI card?
