I first got BF4 about 6 months after it was released and had all the problems you had, no matter what settings...if anything it was even worse on lower settings. 
After all the patching that has gone on recently I have been able to play game, finish a round without crashing and with a fairly smooth steady fps though not at 60 fps.
Not a huge fan of BF4, especially after having so many problems making it unplayable. Battlefield 3 felt much more polished a few months after release.
