It comes with one 6+2 pin to a 2 x 6 pin lead and a 6 pin to two molex, IIRC.
It definitely comes with the first lead that I mentioned as when I fitted that very card to a PC which used a 800w OCZ EliteXStream 800w PSU (which has four 6+2 leads and can delivery 62amps) I needed to remove the 6+2 lead from the 8 pin socket on the card and fit the first lead and then plug in to it 2 6+2 leads from the PSU. This helped the stability of the card. Even though the PSU is rated enough to cope with the power demands I found it better with the two leads connected to the 8 pin socket than the 1.
