Black Screen / PC Lockup

This had started happening about 2 months back when I had my 7950s. I thought it was a graphics card issue and upgraded to the INNO DHS 780 and it is still happening so I believe its probably something else hardware or software related.

Any advice?

Ill list the specs for people
Corsair AX860
Gigabyte GA-Z77-UD4H
Intel 3770k (not OCed)
H100i
INNO 3D DHS 780
Samsung 840 Pro
1 old HDD (could be the problem)

Is Windows installed on the SSD?

Yeah.
It also seems to only happen during games. Ive played League of Legends and F1 for the past month or two and it happens then at random times.

Anyone got any ideas? Seems like it only happens in League of Legends.
Will see if anything happens in watch dogs when its released.
