Boosting Wifi Signals

I am looking at increasing wifi performance, at the moment I can barely struggle to get 1 bar in some parts of house, I was thinking of getting a usb adapter with an largeish antenna, do these actually work, I have been looking at ones for sale and they seem to be 1000mw, 800mw 500mw, what do these numbers actually mean?
 
It sounds like access point and setting it up as a repeater? yes they do work, make sure you can get the right security level in that mode e.g. my old skool APs only work on WEP, in repeater mode.
