Causes of Graphics Card Glitching

What could cause graphics card glitching (apart from the obvious the card is dying)....?

- Overclocked CPU?
- Underpowered Card?
- Overheating Card?
- Bad Drivers?

Mixture of overclocking and overheating for me, if it ever glitches I know I need to turn up the fan or turn back the clocks a bit (not quite done with them as I do still occasionally get glitches after long gaming sessions - though minor)

Still waiting for my fan controller which should hopefully eradicate this problem and allow me to clock higher.
 
It started with BFBC2. Lots of white little squares everywhere. Ran the Radeon overdrive test. Failed. Dropped CPU clocks to stock (weren't very high anyway). Test passed Thought it had fixed it. Squares came back today. Ran test again, failed. Rebooted. Test passed. Ran CS:S stress test, no visible glitching. Ran Radeon test, failed.

Card had always been at stock. What should I try next?

System is Q6600, Core 2 Quad. Gigabyte DS3-R P35, 4GB Cosair DDR2 Ram, Radeon 4870 1gb. Power supply is (I think) 450w, but its a good brand - can't remember which one though.

Drivers? Have just installed 10.2 (was on 9.12). *Seems* to have fixed it good and proper.

Well I don't know. I thought it was ok yesterday, but it started again not long after. Need to play for a bit longer.. I googled Grey Screen of Death. Seemed to talk about 5xxx cards.
