Kepler brought in support for 4 monitors from 1 GPU, so you're pretty much looking for the cheapest Kepler or Maxwell card with 4 individual outputs. Looks like (brand new) that's the Gigabyte 750 at there moment:

Unless the PC can run a single screen from any onboard output, or a USB3 to video output, then a cheaper card could run the additional 3 screens.
