Does anyone know why this may not transmit phone tones during a call?
I am not sure whether the issue lies with the ATA or the service provider but during calls I sometimes find that they ask for various keys to be pressed and the result is always a disaster. I assume that no tones are heard on the other end of the call.

Ah, it seems that it is in the ATA.
Having said that, the configuration in mine seems to be correct, perhaps it is just something to do with the 191? I am not sure now. Perhaps I should try a different ATA.

I had the same thing many many moons ago with an ATA 186.

I'm using a Grandsteam HT801 thee days and DTMF tones work fine. Another nice thing about the HT801 is that as it's USB powered Im using a PoE adapter to power it.

I found it ( hours after I bought a Grandstream ).

The Cisco has an automatic DTMF and it frequently gets it wrong. It needs "DTMF Tx Method" set to "In Band" in the line configuration.

Works a treat!!

Oh good grief, now it's working with HSBC it's no longer working with my ISP.
I'm going to setup the 801 today to see if that's better.

In passing, I found out how to set the cadence in the Grandstream to simulate an old BT ringtone. I discovered that the cadence can be set as a pattern. I have started with c=400/200-400/2000; I am sure that with some fiddling it could be got the same as the old-fashioned ring. If you happen to have old phones like I do, this is important stuff!!!
The 2000 is necessary as well (for reasons I don't understand), without it the caller ID will not be displayed on the phone.
