As per the thread title really, what's the most effective way to do this?

Assuming it's a modern TV so won't have older style connections.

1) rca audio out, to a headphone amp
2) Optical digital out to a DAC/headphone amp
3) HDMI out to avr with headphone stage.

I've used a portable DAC/headphone amp with a Panasonic TV, that works fine. Probably best method as it bypasses the TV's analogue output.

HDMI audio extractor.

Something like this:

About £20?

My LG TV allows to use the SCART socket for audio out for what its worth as it has no other audio outputs.

Above will work if there's only HDMI to work with. Along with something like a SMSl SD793II, will be cheaper and much smaller than buying an AV receiver just for headphones.
