Continual monitor resolution refresh problem

Hi there,
Having 2 problems with my display, not sure if they are connected.

1. Ever since I got my current monitor (Samsung 24" 2494HS, connected by DVI), I've had an occasional problem where the monitor is not detected by bios when first booting, so I just get a blank screen. Hitting reset sorts this. Happens on maybe 5% of boots.

2. Recently upgraded display driver to 197.45. Since then, the system boots to the desktop, but then continually resets the display resolution. When this happens, the screen goes blank, shows no input signal for a second, then comes back on. The desktop then displays for a few seconds, then it happens again (and again and again...). The longest I've left it before shutting down is about 10 minutes. All the while, the hard disk is accessing constantly, and in between resets the cursor turns to the busy icon if held over the taskbar. A couple of times I've also had a message saying that rundll has stopped working.
Can boot fine into safe mode (where I'm writing this now), but if I try booting using 640x480 it will get to the desktop, then change resolution to 1920x1080 and the problems start again. Have tried reinstalling the Nvidia driver - no change. Could it be something to do with the Nvidia control panel trying to over-ride windows settings?

Any advice?
 
Is the cpu usage running at 100% or near it all the time when the machine is idle? Does the hard drive still access all the time even when in safe mode? When I get a pc to fix with hard drive accessing constantly it was often down to spyware/viruses but as I don't work on as many Vista pc's as XP machines this is only a guess.

As for the monitor problem did you remove all Nvidia drivers fully? I do know there are Nvidia driver removal tools available BUT I can't recommend any as it's been a while since I had problems with my Nvidia Drivers. Did you remove the drivers in safe mode?
Hopefully someone else will post the best way to be certain you have removed all traces of old Nvidia drivers. I don't play games that often now so I don't need to upgrade drivers regular. :)
 
Think I may have identified the problem - unmuted the sound and realised that the new hardware connected / disconnected sounds were playing constantly - think I must have a loose connection / fault somewhere in the graphics card, DVI cable or monitor. Have switched to an old LCD panel connected with VGA, working fine. Now will have to start testing!
