Corsair Dominator-GT Module Dead!

Need some advice from Yellowbeard please!

I have a stick (one of three) of Dominator-GT TR3X6G2000C8GT which has died.
The bios reports just two sticks and while MemTest86+ reports the slot is occupied it can't test it!

I've been looking at the RMA section of the Corsair site and the module doesn't seem to be listed! How do I get round this?

Do I have to return all three modules?

The part number has been changed to CMG6GX3M3A2000C8. Try switching the modules around in the board so that the suspect module is in a known working MOBO slot. It's possible you have a bad slot or bad memory controller on the CPU.

If you decide you need an RMA, use THIS LINK and we'll replace them ASAP. Return all 3 modules.

I inadvertantly checked the slots when trying to establish which module was faulty.
 
Should I use the (bulkier) original packaging?
Do I also need to return the cooling fans which were part of the original purchase?
 
Well I duly sent it off last Monday the 18th and it was duly delivered Wednesday morning. I had an email Thursday confirming receipt for RMA
1144150. The email also mentioned to allow for an additional 5 working days.
So when, roughly, should I expect to hear something?
Also, are there shortages of this particular module since I note OC are out of stock.
