Crashing and constant hard drive use with XP Home

A friend asked me to fix their computer and from what they said it sounded simple enough but when I looked at it boy was it screwed.

When it boots up it runs very slowly and the hard drive is constantly being accessed and after being up for a very short period of time it restarts (immediate reboot NOT shutdown it just flicks straight to POST). I've brought up task manager and the CPU isn't being used that heavily, I've taken the hard drive out and frun chkdsk /r and virus scanned it all of which produced nothing. I ran adaware on it as I was running short of ideas and that turned up a few things but none of which would cause this.

Does anyone have any ideas of how to proceed other than a format and reinstall?
 
its probbley blue screening but the person has got the "restart PC"

forgot what its called now, its in System -advanced - startup and recovery,

see what the STOP error is, or check event log which will probbley be your best bet.
