Custom Case ventilation

Just wondering if anyone's tried cutting a hole in the top of their case roughly between their PSU and the 5 1/4 bays and stuck a fan in to see if you can suck some more air out of the case or blow in for that matter. Think it would make any difference?
Have a look at the Pic below, and yes i know its a bit of a mess!! but then you don't see in my case where i have it.

I did see a budget make case on another site that has a 10inch fan on the side of the case, dunno how fast it spins or what kinda noise it makes but i'm guessing as long as there's sufficient exhausts to blow the air out again it should be one very efficient way of cooling everything!

cos it's a sata drive and it has the sata power cord plugged in.
