CV/Job application help

Got a letter off the job center when i was there last for a job that id like to apply for and it says "Please forward your CV to:" not sure what i do, do i just send my CV and the form the Job center gave me with all the job details in or should i just print something out saying im applying for the position and this is my CV. not very good with this stuff

Usually a covering letter, yes.
