Do you personally include them or not?

Do you use "Available upon request" or do you leave that off?
 
don't put anything IMO

about as useful as writing curriculum vitae at the top in a large font - tis just a waste of space

If they want a personal reference, they'll ask for one, if they want to contact former employers - you've got them listed on the CV anyway

lots of places outsource background checking too these days - you could be given a form and be asked to list all employers for up to the past 10 years + account for any gaps of greater than 1 month - someone at the background checking place will then check each of these

Never put any contact details of the referees on the CV. You cna put "available on request" but that doesn't add much value.

I leave it off the resume and in the personal covering letter I write to each company state that I have references.


Also in general I have never heard of a company use references before interviewing someone. Normally they look at the CV, if it looks good then they interview the candidate, perhaps with a phone/skype interview and then personal interview. Once all of that is done and there are some remaining issues or doubts then the referees are contacted looking for confirmation (one way or the other).
 
They shouldn't be included as they are less likely to be harassed then.

Only after an interview that went well would I provide the references.
