Saw a YouTube video recently.
Basically, damage occurs when you push the disc as it's going in and when you pull the disc as it's coming out. Doing either motion damages the rollers and maybe other bits.
So to avoid damage, just gently push the disc in until it takes the disc and wait for the disc to be fully ejected before pulling it out.
One of my grandson's managed to insert 5 disc's in the ps4 we keep upstair's for when the grandchildren stay,so they can watch a film before going to sleep.
I looked on youtube and took the ps4 apart myself(i am 63,with not the best eyesight),and got the disc's out,re-assembled,and is working as new now,some 7/8 months later,so give it a go,nothing to lose,and good luck
