Hey guys,

My girlfriend is doing her dissertation on social networking sites and would appreciate it if you could fill out this very short questionnaire.

Ok, done it. If the questionnaire could be redesigned then I'd suggest bigger text boxes so that it is easier to review what you've written rather than having one long stream.
 
Done, but couldn't think of anything relevant to write in the "Why?" boxes so i improvised.

Thanks very much guys! I think she just used a questionnaire website to generate it. The aim was to keep it short enough for people not to get bored.
 
Done, was quick :)

You new students have it easy though, questionnaire's used to be on paper and we used to have to walk around attempting to get people to write on them

Don't shoot the messenger! I done BA (Hons) Accounting so I didn't have to do any of this dissertation rubbish :p Paying for it in professional exams though :(
